To get started, simply fill out the short form below and we will e-mail you the application as an Adobe PDF file. Once completed, simply mail or fax it to us to begin processing your application. Within 72 hours, you will be notified if your practice has been approved. This short application will allow us to better understand you and your practice, including validating that you are a board certified or a board eligible plastic surgeon by the American Board of Plastic Surgery. CosmeticProtect is not offered to non-board certified physicians performing plastic surgery procedures.
